5 NFRA Inspection Findings That Audit Automation Prevents
The National Financial Reporting Authority publishes inspection reports on audit firms and individual auditors. These are public documents. They reveal, with specificity, where Indian audit practices are falling short.
Reading across NFRA's inspection findings, a consistent pattern emerges. The same deficiencies appear across firms, across engagements, across years. They are not random failures — they are structural weaknesses that arise when manual audit processes face data volume, time pressure, and documentation demands.
Audit automation directly addresses five of the most frequently cited categories.
Finding 1: Insufficient Audit Documentation
What NFRA finds: Working papers do not sufficiently document what the auditor did, how they did it, why they concluded what they concluded, and what evidence they relied on. SA 230 requires documentation that allows an experienced auditor with no prior engagement connection to understand the audit work performed. NFRA routinely finds that files fail this test.
Why it happens manually: Documentation is the last step of a time-pressured engagement. When an audit file is running behind schedule, documentation suffers. When article clerks prepare documentation, quality is inconsistent. When a partner reviews the file days after the fieldwork, gaps are difficult to identify and fill.
How automation prevents it: Automated audit systems generate documentation as a direct output of testing — not as a separate step afterward. Every flag is linked to the voucher that triggered it, the rule that tested it, and the regulatory authority behind the rule. The working paper is populated automatically from test results. There is no separate documentation step that can be skipped.

Finding 2: Inadequate Professional Skepticism in Related Party Testing
What NFRA finds: Auditors accept management representations on related party transactions without independent verification. Related parties are not independently identified — the auditor relies on the list provided by management. Arm's length pricing is not tested against market benchmarks. Disclosures under Ind AS 24 / AS 18 are not verified against the auditor's own identification.
Why it happens manually: Identifying related parties independently requires cross-referencing entity structures, director shareholding patterns, MCA filings, and transaction patterns — a significant data exercise. Most manual audits simply verify management's list against disclosures rather than independently constructing the list.
How automation prevents it: AI-powered related party identification analyses transaction patterns to surface vendors, customers, and counterparties that have structural or transactional characteristics of related parties — regardless of whether management disclosed them. The AI flags transactions between the auditee and entities that share directors, shareholders, or addresses.
This does not replace the auditor's judgment on whether a flagged party is indeed a related party. But it means the auditor is not relying entirely on management's self-reporting of its own related party relationships.
Finding 3: Sample Sizes Too Small to Support Conclusions
What NFRA finds: Auditors draw conclusions about account balances or transaction populations based on samples that are statistically inadequate. The sample is too small to support the conclusion that no material misstatement exists. This is particularly noted in areas like vendor payment testing, journal entry review, and revenue recognition.
Why it happens manually: Larger samples mean more CA time. For a firm with 50 audit clients and time-pressed article clerks, the practical constraint is not statistical theory — it is available hours. Samples shrink under time pressure.
How automation prevents it: When transactional scrutiny runs on 100% of the population, sampling risk is eliminated entirely. The auditor's conclusion that "no material misstatement was found" is based on 100% coverage — a position that is statistically unassailable.
The SA 530 requirement to document sampling methodology, sample size determination, and selection method becomes irrelevant when the entire population is tested. NFRA cannot criticise a sample size of 100%.
This does not mean vouching 100% of documents — physically reviewing every voucher remains time-intensive. But ledger scrutiny, TDS reconciliation, and GST reconciliation can all run on 100% of transactions in minutes.
Finding 4: Risk Assessment Not Linked to Audit Procedures
What NFRA finds: The risk assessment (identifying significant risks, assessing inherent risk and control risk) exists as a separate document that does not visibly inform the audit procedures actually performed. Auditors complete a risk matrix and then conduct a standardised audit program regardless of the risk profile. High-risk areas do not receive more extensive testing than low-risk areas.
Why it happens manually: Building a genuine link between risk assessment and audit procedures requires more judgment and more documentation than filling in a standard program. Under time pressure, the risk assessment becomes a compliance checkbox rather than an audit planning tool.
How automation prevents it: AI-driven risk scoring assigns risk levels to individual transactions based on multiple factors — amount, timing, counterparty, narration, pattern. The highest-risk transactions surface automatically for auditor attention. The audit procedures applied are proportionate to the assessed risk — not because the auditor remembered to do so, but because the system prioritises exceptions by risk level.
The working paper documents the risk basis for each procedure. The link between risk assessment and procedures is automatic and traceable.
Finding 5: Inadequate Response to Going Concern Indicators
What NFRA finds: When financial data contains indicators of going concern risk (declining revenue, negative working capital, debt covenant violations, significant losses), auditors do not sufficiently document their evaluation of management's plans and their independent assessment of whether the going concern assumption is appropriate. In some cases, auditors sign clean opinions on entities showing clear going concern stress without adequate documentation of their evaluation.
Why it happens manually: Going concern evaluation is a judgment call — but making a well-documented judgment call requires assembling financial trend data, reviewing management's mitigation plans, and documenting the rationale for conclusion. This documentation takes time that is often compressed at engagement close.
How automation helps: While going concern assessment remains a professional judgment that cannot be automated, AI can flag the indicators that require evaluation — negative net worth, cash flow deterioration, debt service coverage ratios below covenants, consistent loss-making, significant decline in key revenue lines. These flags ensure the auditor addresses going concern explicitly rather than overlooking it.
The documentation framework ensures the going concern evaluation, once made, is recorded in a structured way that satisfies SA 570 requirements.
The Common Thread
All five of these NFRA findings share a structural cause: manual audit processes, under time and resource pressure, produce inconsistent, incomplete, or insufficiently documented work.
Audit automation does not make auditors better at professional judgment. What it does is:
- Eliminate the time pressure that causes documentation shortcuts
- Ensure 100% population coverage where sampling risk is a concern
- Generate consistent, traceable documentation automatically
- Surface risk indicators that manual review misses
- Create a documented audit trail that survives NFRA scrutiny
The CA's professional judgment — evaluating the exceptions, assessing going concern, applying materiality, evaluating estimates — is unchanged. The mechanical work that surrounds and supports that judgment is handled by the system.
SQM1 and Firm-Level Quality
NFRA's inspection findings are increasingly examining not just individual engagement quality but firm-level quality management systems. From July 1, 2026, ICAI's SQM1 standard is mandatory for all audit firms. SQM1 requires firms to document quality objectives, conduct Engagement Quality Control Reviews for significant engagements, and maintain ongoing monitoring records.
