Root Cause Analysis — ___

Period / scope: ___ | Prepared by: ___ | Reviewed by: ___ | Date: ___

Use this form to move from a finding to its cause, then to an action that stops it recurring. Ask why until you reach a cause you can change. Stop at a cause the organisation controls, not at "human error".

Common questions

FAQs.

When should I use 5 whys instead of a fishbone?
Use 5 whys for a single, fairly direct problem. Use a fishbone when several causes may combine and you want to map them before testing which are real.
How do I know I have reached the root cause?
When the cause is something the organisation controls and removing it would stop the problem recurring. "Human error" is rarely the root.
